If you are scheduling appointments on behalf of your client, please include a call back number so we can collaborate to determine time and date for an appointment. Once this form is submitted please inform your client that we will be reaching out 24-48 hours to touch base. Once the referral is received on our end, they will get an email to login to our portal. They will need to sign consent forms, input demographic information and their insurance information before the first session.
